Confirmed reception points: 8Documents and requirementsAfter an official name change, plan passport replacements and updates to related records.

This route describes the general procedure. Some steps depend on your circumstances and may involve different authorities. Check the current requirements for each service before applying.
This route starts once you have formal evidence of the change, such as a marriage or name-change certificate. Follow only the subsequent procedures relevant to the documents you hold.
Check the new spelling and dates in the supporting document. If the change is not yet official, first ask the civil registry about the appropriate procedure.
Choose ID-card exchange for an existing ID, or the appropriate booklet-to-ID procedure. Submit the name-change evidence and the national passport being replaced.
If you hold an international passport, check replacement and consistent Latin spelling. Ask the chosen office whether both passports can be processed together.
Check how to update taxpayer details. Follow the procedures of your bank and other organisations that use the previous details.
Prepare your updated identity document, existing licence and name-change evidence. Check the available Ministry of Internal Affairs, Diia or confirmed CNAP route.
Check your sole proprietor details in the Unified State Register and apply for changes if needed. A passport replacement does not confirm that every business record has been updated.
Tick the documents you have prepared. This is a general checklist: items marked “If applicable” are only needed in the relevant circumstances. Check the service requirements for the final document list. You do not need to upload documents to this website.
The “A name change and your documents” route connects several applications. Open the relevant procedure for your next step and check where to apply; optional actions depend on your circumstances. Checklist ticks help you organise preparation and do not confirm that an institution has completed an action.
