When to request a replacement
A replacement Ukrainian birth certificate is issued using an existing civil-status record. This route can be relevant when a certificate has been lost, stolen, damaged or destroyed, or another prescribed ground applies. It is different from registering a newborn for the first time. If the underlying record is missing or needs correction, ask the civil registration authority, known as DRACS, about that separate procedure.
Who is entitled to apply
Decide whose document you need: your own, your child’s, or another person’s for whom you are entitled to act. In addition to identification, the application may require evidence of a family relationship or authority to represent the person. Knowing somebody’s name and birth date does not alone establish a right to obtain their certificate.
Civil registry offices, CNAPs and digital availability
The primary in-person route is a DRACS civil registration office; confirm acceptance before choosing a particular CNAP. The Diia portal page previously stated that online replacement birth-certificate applications are temporarily unavailable. Plan digital submission only after the official service confirms restoration. Check collection and any delivery arrangements with the channel you choose. For applications from abroad, separately check the Ukrainian consular route.
Certificates, extracts and checking the document
Before paying, confirm which document the receiving organisation needs. A birth certificate and a birth register extract are different outputs. Review all personal details when the document arrives. For use in another country, ask about authentication and translation requirements for that purpose; the rules are not identical for every destination. Keep the application confirmation so you can follow the existing request rather than submitting it again unnecessarily.
Information to organise for the record search
- Known names, date and place of birth, and any civil-record details.
- Applicant identification and any required relationship or authority evidence.
- The exact result requested: a certificate or an extract for another procedure.
Common questions
Must a birth be registered again if the certificate is lost?
If the civil record already exists, check the replacement certificate procedure rather than registering the birth again.
Is a birth register extract the same as a certificate?
No. Check which document the receiving organisation requires before applying.

Re-registering a child's birth or changing the record
A lost certificate, changed details and an absent civil-status record are different cases. If the record exists, losing the paper calls for a replacement certificate, rather than registering the same birth again.
Correcting or changing details, including the child's parentage, requires the appropriate DRACS procedure and a valid basis. The Justice Ministry's official explanation identifies an application by an entitled person, identification, the certificate containing details to be changed, and supporting evidence. The legally entitled parent or guardian applies for a child. A replacement certificate follows an approved change; asking to “re-register” does not itself establish a legal ground.
If the record itself is missing, restoring it requires documentary confirmation of its absence. Losing the paper certificate does not establish that the register record is missing. Explain which situation applies before choosing the procedure.
