A passport is lost or stolen

Choose the correct ID-card or international-passport replacement route and prepare the supporting evidence.

A Ukrainian biometric passport with a boarding pass

This route describes the general procedure. Some steps depend on your circumstances and may involve different authorities. Check the current requirements for each service before applying.

Getting started

Identify the missing document: an ID card, a passport booklet or an international passport. Different procedures apply. This route helps you prepare to apply in Ukraine.

What to do

  1. Identify the document and report the loss

Promptly ask the migration service how to report the loss. For a passport booklet, confirm the separate requirements; the ID-card replacement service applies to a missing card.

  2. Report theft to the police

    If applicable

If the passport was stolen, report it to the police and obtain the prescribed criminal-investigation register extract. Describe a loss accurately rather than reporting it as theft.

  3. Gather available documents

Prepare the tax number, any other available identity document and evidence of additional details. If none are available, discuss identity verification with the migration service in advance.

  4. Apply for a replacement

Choose an office confirmed to accept the relevant replacement. Check fees, payment details and attendance requirements, then check the application before signing.

  5. Collect the new document

Keep the application details and use the official progress-checking channel. If the old passport is found after invalidation, do not use it; notify the migration service.

Document checklist

Tick the documents you have prepared. This is a general checklist: items marked “If applicable” are only needed in the relevant circumstances. Check the service requirements for the final document list. You do not need to upload documents to this website.

Things to consider

  • For a passport lost abroad, contact the competent overseas authority; this route does not cover consular applications.
  • Identity verification may affect timing. A CNAP appointment does not establish that every passport procedure is available there.

Continue to individual procedures

The “A passport is lost or stolen” route connects several applications. Open the relevant procedure for your next step and check where to apply; optional actions depend on your circumstances. Checklist ticks help you organise preparation and do not confirm that an institution has completed an action.
