Fee

UAH 618 for standard processing or UAH 988 for urgent processing under SMS fees from 1 January 2026. A separate state duty may apply after loss; confirm exemptions and payment details before paying.

Processing time

Up to 20 working days, or up to 7 working days for urgent processing according to SMS. Additional identity verification may require a different timeframe.

Who can apply

A Ukrainian citizen who lost an ID-card-format national passport or reports its theft.

Documents to prepare

  • Application form; a pre-trial investigations register extract after theft.
  • Tax number or documented lawful refusal of a tax number.
  • Confirmation of the administrative fee and state duty, or an applicable exemption.
  • Available residence, marriage, children’s birth and name-change documents, and an IDP certificate.
  • Representative authority or a 10 × 15 cm photograph for prescribed special arrangements.

How to apply

  1. Confirm that the chosen office accepts this particular passport application and check its appointment procedure.
  2. Present original documents; the officer prepares the application, takes a photograph and collects the required biometric data.
  3. Check the spelling of your name, keep the application details and collect the completed document under the office procedure.

What you receive

A Ukrainian national ID card or a reasoned refusal.

  • Missing documents or documents that do not meet the requirements.
  • Identity, citizenship or the supplied information cannot be verified.
  • The representative has no documented authority.
