What the Ukrainian residence extract confirms

A territorial community register extract records declared or registered residence or place of stay, deregistration, or the absence of such information at the time it is generated. Another administrative procedure may require it. It is not a tenancy agreement, proof of ownership, an IDP certificate or an application to register a new address.

Searches for “Form 13”, “Annex 13” or a “propiska certificate” often refer to the former certificate under Rules No.207. Those rules and the certificate forms in annexes 13 and 16 ceased to apply when Procedure No.265 took effect on 14 March 2022. For new proof of address, use the current residence-extract service.

Requesting an extract through Diia

Open the official service, sign in and check your details. Select an extract for yourself or a child. For a child’s extract, the portal asks for the birth certificate’s series and number. Download the completed PDF from the received-documents section. The service is free.

In the Diia app, find the residence-extract service among certificates and extracts. Its exact menu position may change with app updates. You can save and print the PDF: paper and electronic versions have equal legal force. There is no need to copy the information into a third-party blank extract.

Applying at a CNAP or through a representative

For an in-person request, use the registration authority or CNAP of the relevant community. The basic documents are an application and an identity document or certificate of application for protection in Ukraine. Children’s applications and representation require the relevant additional documents and authority. A family relationship alone does not grant unrestricted access to personal records.

Consult official service card 00038 and obtain the current application form from the registration authority or its official website. Enter your own details and the information requested. The system generates the extract; the applicant does not fill in the finished document.

QR verification and validity

The extract carries a unique number and a QR code. Scan the code and compare the verification result with the document. An edited picture or someone else’s specimen is not a substitute.

The extract represents the register at its generation date and time. It has no fixed general expiry period, but an old extract will not show a later change of address or personal details. If an institution requires information as of a newer date, clarify the basis and generate a current extract. Do not assume a universal 30-day validity rule.

Missing records and occupied territories

Check your personal details and use the official register-update service or contact your community’s registration authority. Failure to generate an electronic extract does not by itself prove that an address was never registered.

Diia specifically warns that extracts are unavailable for registrations in Crimea and parts of Donetsk and Luhansk regions occupied in 2014–2015, because those records were not included in the community register. Check the legally accepted booklet passport or electronic passports containing the information instead. Do not automatically apply this specific limitation to every address occupied later.

A move requires separate address registration. Find the relevant community in the CNAP directory.
